Tell Congress to Support the LTC Pharmacy Fix in 2025

LTC pharmacies are the core pharmacies that support nursing homes as well as assisted living facilities, and other long-term care settings. New Medicare Part D drug pricing policies will soon jeopardize the ability of many LTC pharmacies to keep their doors open, providing essential, specialized and legally required pharmacy services to seniors and other LTC patients. A recent SCPC member survey shows that without Congressional action more than half of America’s LTC pharmacies, would be forced to close pharmacy locations.

Soon to be implemented government mandated Medicare drug pricing policies will significantly impact my long-term care (LTC) pharmacy, making it difficult or even impossible to keep providing essential, specialized and legally required pharmacy services to seniors and other patients in nursing homes and assisted living facilities.

The Senior Care Pharmacy Coalition (SCPC) recently released a member impact survey on the unintended consequences of Medicare Part D price negotiation policies included in the Inflation Reduction Act (IRA). The results are alarming, as more than half of America’s LTC pharmacies would be forced to close pharmacy locations if Congress fails to act.

SCPC pharmacy members represent 75% of the LTC pharmacy sector and serve about 850,000 patients daily in skilled nursing and assisted living facilities across all 50 states. Without a solution before January 2026, when negotiated prices take effect, SCPC’s LTC pharmacy members will suffer the following repercussions:

• 60% would be forced to close pharmacy locations.

• 91% would be forced to lay off pharmacy staff.

• 85% would be forced to limit essential services.

• 82% would be forced to shift costs to LTC customers.

• 56% would be challenged to dispense certain medications.

Medicare Part D and PBMs force LTC pharmacies to subsist under an inadequate payment system.

Because LTC patients rely on 8 of the first 10 drugs subject to Medicare-negotiated prices and these medications are significant to LTC pharmacy survival, these pharmacies will face unintended but detrimental impacts from dispensing MFP drugs when new prices take effect in January 2026.

Fortunately, there is a solution to mitigate this issue and make sure that LTC pharmacies are not unnecessarily harmed. The LTC Pharmacy Fix would create a limited two year $30 LTC pharmacy supply fee to be paid each time an MFP medication is dispensed, ensuring LTC pharmacies can continue providing essential care to millions of patients in LTC nationwide in the first years when their major medications are being impacted by MFP changes.
